| BILLY MOCK was a loving Christian with a dynamic enthusiasm for life. His infectiously positive attitude, ever-present smile and vibrant personality literally impacted the lives of thousands of people. Billy began fighting brain cancer in 2005 and touched thousands more lives until his death in 2007 at the age of 13.
The Billy Mock Foundation is a tribute to Billy and his compassion for people, love for life, and passion for sports. Our mission is simply to “help others” which is achieved through the award of scholarships and support of families in need. The Billy Mock Foundation (a 501(c)(3) non-profit organization) was established in 2008, on what would have been Billy’s 14th birthday, by The Mock Family and Friends. It has already awarded several scholarships, provided financial support to many families and instituted a youth-focused pay it forward program and a junior board. |

| So why the National Kidney Foundation?
In 2002, my then 20-year-old brother, Justin, was diagnosed with kidney failure. For the following five years, Justin was in and out of doctors offices, traveling to different states, changing his diet, searching for anything that could save him. However, the inevitable seemed to be on the horizon – he would need a kidney transplant. Friends and family were all tested to see if they were a match in order to be a donor for Justin. We all crossed our fingers and one donor stood above the rest – our sister, Jenna, would be the one to to save Justin’s life and give him a second chance. In 2006, Justin had a kidney transplant thanks to Jenna, his donor. It was a tough five years leading up to the surgery, and even tougher weeks of recovery to follow post-surgery. But here we are, celebrating the five year anniversary of Justin’s kidney transplant! Justin is strong, healthy and takes nothing for granted! So much of this journey was made possible due to the work done by the National Kidney Foundation (NKF). The NKF’s hard work, research and determination deserve credit for the reason I have my brother with me today! The Annual Gdovin Designs Charity Golf Outing is set up for one reason – to give back to the organization that gave Justin so much! We’re trying to combine having a great time with helping a great cause. Lots of fun, great food, plenty of drinks and lots of prizes! |

| Hope’s Door is a shelter and counseling center that offers comprehensive programs and services that are designed to help break the cycle of domestic abuse. Hope’s Door provides direct intervention and prevention services to families affected by domestic abuse in Collin County, Dallas County, and surrounding areas.
When Hope’s Door first opened its doors in 1986, the agency operated a 24-hour crisis line, but did not have the space to provide shelter. Clients were housed at local hotels. In 1989, the agency opened its own shelter and began providing temporary housing to victims of domestic abuse. Since 1989, Hope’s Door has provided safe shelter for over 3,787 women and their children, and answered over 16,000 crisis line calls. We are very excited about our event this year and hope that it is a success. |

19th Annual Michigan NARSAD Golf Classic raises donations for the Brain & Behavior Research Fund. Pep’s love for family, drove him to raise funds that will lead to better treatments and cures for millions who suffer from serious mental illness. NARSAD researchers are discovering new things about the brain & behavior disorders, including schizophrenia, depression, bipolar disorder, autism, epilepsy, suicide prevention, PTSD, anxiety and stress, panic disorder, phobias, ADHD, Alzheimer’s, Fragile X, Parkinson’s, SAD, drug abuse, insomnia. etc. See videos at http://www.narsad.org/?q=node/11143/multimedia. | The PHA Charity Golf Event takes place June 27th at The Wilds Golf Club in Prior Lake, with a shotgun start at 1pm. Golfers will play in foursomes in an 18 hole scramble. Invite 3 friends to make your own foursome, or we’ll put individuals into teams prior to the event.
Peace House Africa’s sole function is to raise funds for and awareness of Peace House Secondary School (PHS), a boarding school in Tanzania for AIDS orphans and vulnerable children. Instead of a registration fee, we ask each golfer to raise a minimum of $1,500 through family, friend and associate contributions. All you do is send us your list of potential contributors and play in the event, and we’ll do the rest of the work! Peace House Africa staff sends emails to your contacts telling the story of a child at PHS. You and your donors have the unique opportunity of getting to know one of the students in Tanzania you’re supporting! Your participation profoundly impacts 280 deserving students striving for education at PHS. Please join us for this fun, meaningful day on the course. With your generosity and support, we look forward to a successful event! |

| The Rich Velasquez Youth Sports Equipment Foundation was founded to honor one of the Lakes Regions more avid youth sports supporters. Rich was active in many of the cities youth sports programs. He was an active Baseball Umpire of Laconia Little League and upper levels of play. Rich enjoyed umpiring and working with the players. He got to know many of the players on a personal level and they enjoyed carrying on conversations and getting to know him. While recruiting new players for tee ball, he would share info with his wife Cyndy. As part of the head start program in Laconia, she knew of families that would love to play but did not have the proper equipment. She would tell Rich about these families. Then when she got home the next day, there on the table was the equipment the family needed to play. That is what Rich was about – Not wanting to be recognized for his efforts and being around, watching everyone playing and being active. T hat is why this foundation will make every effort to be sure no child is left out due to a lack of equipment or any other monetary issue.
In June of 2008, Rich headed to the hospital with stomach pains. Finding out that he had gall stones was the start of what seemed to be major organ failure. After battling his health problems for 5 months in ICUs at 3 different hospitals, Rich passed away on November 15th, 2008, having had problems with his Kidneys, controlling his Heart Rate, and Lungs. |

| We are extremely excited to announce the 1st Annual Fire Buck Charity Golf event! This event will be held at The Links at Groveport in Groveport, Ohio on Saturday, August 27th 2011. Tee-time will begin at 8:00a.m. and last until 10:20a.m., with room for 60 golfers (15 teams of 4). We will be using a new ‘Half-Scramble’ format to facilitate game play, without rushing through the course.
Also included with this event will be a lunch buffet, drinks, and a 50/50 raffle. We are also working on a potential silent auction, in which all proceeds will go to charity as well. If you are able to contribute to the silent auction, please let us know. Gift cards and Gift baskets are our most desirable items. Proceeds will benefit a local Teen Suicide Prevention Center in the Greater-Columbus area and will be donated in memory of Leah Piteo, who lost her life on Tuesday, April 12th 2011.
