| For sixteen years we have been humbled by the kindness and generosity of individuals and businesses alike, who understand the importance of supporting the “The John L. McGuirk Memorial Golf Tournament”. Over $120,000 has been raised to benefit medical research and treatment of high-risk pregnancies at Massachusetts General Hospital Vincent Service and the North Shore Cancer Center’s Wellness Fund, which helps to counsel and support children of families who are coping with the ravages of cancer.
Each year is more difficult than the past year(s) to garner support. With the tough economic times, everyone’s dollar is stretched. However, please remember that there are no economic boundaries for Cancer or High-Risk Pregnancies. They can strike anyone at any time, without warning. We know first hand, we lost someone very special to all of us 8 years, Coach John L. McGuirk. He was a husband, father, coach and friend to everyone. And a family member lost two babies at birth 15 years ago. Both are insidious diseases that taxes families in countless ways; physically, financially, emotionally. There is no more hopeless feeling than watching someone that you love struggle with this singularly devastating illness. This is why once a year we gather in solidarity to help others, while spending an enjoyable afternoon on the links. So you may ask, “What can I do?” There are many ways to contribute to this endeavor. We offer Sponsorship Packages ranging from $75.00 to purchase an ad in our program book to $3500.00 to sponsoring the dinner. With your sponsorship, you company’s name will be displayed throughout our tournament. If you are not a golfer, or are unable to join us, but would still like to partake in this worthy cause, we gratefully accept with pleasure any and all donations. |

| In 2004 our son, Hayden, was born with Hypoplastic Left Heart Syndrome. Over the past six and a half years, he has received amazing care from the Pediatric Cardiology Department at Johns Hopkins Hospital. Each day so many children and their families are cared for and supported by this dedicated team of professionals.
We are thankful for the doctors who saved Haydens life more then once, the Nurses who cared for him and us, the social workers who kept us informed and the child life specialists who attempted to make the hospital seem like a normal place. Mostly, we are thankful for Hayden. In order to give back and allow other children to benefit in the same way Hayden has,we (Ali and Rob Lazorchak) founded the charity, Hayden’s Heart Heroes. Our annual golf tournament benefits the Hopkins Pediatric Cardiology Department and the Hopkins Heart Family Network.Hayden is our Miracle. With your support Hopkins can continue making Miracles Happen for children with congenital heart defects. For More Information please contact: Rob Lazorchak His e-mail is: Roblazor@aol.com |

| Once again, we are proud to present the Boys & Girls Club of Hollywood Champions Charity Golf Tournament. The Boys & Girls Club of Hollywood has touched the lives of children ages 6 to 17 for more than 70 years. The mission of the Club is to inspire and enable young people to realize their full potential. Serving more than 1,600 children annually, the Club’s key programs emphasize education and career development; health and life skills; character and leadership development; the arts; and sports, fitness, and recreation.
It’s hard to believe that 11 years ago the Los Angeles & Southern California Claims Team from Farmers Insurance founded this tournament. We are grateful for the ongoing support from Farmers Insurance and are pleased to announce that Farmers Insurance will again be the event’s Title Sponsor. We are returning to Robinson Ranch in Santa Clarita and will utilize both 18-hole golf courses. As always, each course will host a variety of contests and activities. After golf, participants will have the opportunity to win great chance drawing prizes and auction items at our dinner and awards banquet. Of course, events like this can’t happen without your generosity. We hope you will be able to join us Friday, October 22nd and invite you to consider the different ways you might offer your support. Your company can participate at various giving levels, with attractive benefits at each. Enclosed is a sponsorship form that explains the sponsorship levels in more detail. If you are able to respond by August 15th, we will be able to give you recognition in the invitation that will be sent out at the end of August. |

| For fifteen years we have been humbled by the kindness and generosity of individuals and businesses alike, who understand the importance of supporting the “The John L. McGuirk Memorial Golf Tournament”. Over $100,000 has been raised to benefit medical research and treatment of high-risk pregnancies at Massachusetts General Hospital Vincent Service and the North Shore Cancer Center’s Wellness Fund, which helps to counsel and support children of families who are coping with the ravages of cancer.
With the tough economic times, everyone’s dollar is stretched. However, please remember that there are no economic boundaries for cancer or high-risk pregnancies. They can strike anyone at any time, without warning. Cancer is an insidious disease that taxes families in countless ways; physically, financially, emotionally. There is no more hopeless feeling than watching someone that you love struggle with this singularly devastating illness. This is why once a year we gather in solidarity to help others, while spending an enjoyable afternoon on the links. |
